Register With The Practice?
It is a requirement that all patients wishing to join the practice
complete both an NHS Registration Form and a New Patient Health Questionnaire
before being registered. You will also need to take your blood pressure using
our automated machine in reception before passing the printed result sheet (adding
your name and date of birth on the back) to the reception desk. If you are asked
to provide a urine sample for testing, please place your labelled specimen in
the special box in the foyer.
We can only accept registration papers between 9.00am - 12 noon and 2.00 - 5.30pm
Monday to Friday.
The specimen of urine required as part of the registration process needs to be
delivered to the surgery between 8.00 - 11.30am Monday to Friday.
In addition you may wish to opt for a consultation with a health care professional.
Appointments can be made through reception but please note you will need to have
completed the questionnaire, forms and be registered on our system before seeing
the nurse.
Although you will register with the practice as a body you have
the right to express a preference to receive services from a particular GP which
we will record. Please note that some services are not provided by all
doctors and that in some cases, due to demand for a particular GP or other reasonable
grounds, you may not always be able to see your preferred GP.
For example, please remember we operate two Surgeries with the same doctors and that nat all doctors work full time.
Carers
Are you caring for someone who has a long term health condition?
If so, please let us know. We may be able to give you important help and advice. It will be helpful for the doctor to know that you are a carer as it may affect your own health and treatment.
This practice has a lead worker for carers. Please ask if you would like to talk to our Carers Lead.
When you tell us that you are caring for someone we will place your details on our Carer’s Register so that practice staff are aware you are a carer. We will not share this knowledge with anyone else unless you tell us
that we can. We may be able to offer you other services to help you care for your own health.
We will try to help you if your caring responsibilities mean that you have difficulties attending appointments.
If you are giving regular and substantial help to the person that you care for, you will be entitled to a Social Services Carers Assessment. The Assessment gives you advice and information.
You and the person that you care for may be entitled to practical help and support. Contact the Social Services Department where the cared for person lives to ask for an assessment. This is a free service.

How Do I....
Change My Details?
If you change your name, address or telephone number please notify
the surgery immediately. It is obviously important that our records are up to
date so that we can communicate with you whenever necessary. If you are waiting
for an appointment to be sent from the hospital, please notify the hospital of
your new address. Telephone numbers and postcodes are very helpful in ensuring
that messages can be relayed promptly and efficiently. If you move outside the
practice area you will normally be asked to transfer to the list of another practitioner.
Advice on which doctors have surgeries close to your new address can be obtained
